It means different things to different people, but an example would be if a branch of government refused to adhere to the clearly written or even generally established constitutional norms. For example, if the House subpoenas his tax records and the administration refuses to provide, that would be a breach of the house oversight duties. And if it went to the SC and the court ruled against the executive branch, and the president still refused, that would be a full blown crisis. Nixon could have done this with the tapes in 73, but resigned instead.
