For me it is The Clash in 1980. I was just getting into concerts (I was only 14) and I had no idea they were playing. Luckily, I was able to see them a few times in 1982 and 1984.
In 2nd place is XTC. The singer suffered from stage fright so they stopped touring before I could see them.

Soundgarden on the King Animal tour, 2013. They didn't come to Winnipeg, but they played Minnie. My buddy, a massive Cornell fan, we were going to go together. He had a fucking wedding that same weekend. He was in the wedding party. Only got to see them once in 94.

IDK, "regret" is too strong of a word. They're just concerts. I'm happy with the hundreds of concerts I've seen and my general POV is if I'm happy and satisfied today, I don't have any regrets of the past because what happened got me to this place I'm at today.
I guess I can kinda say Gorge 1 2006 because I travelled all the way out there for night 2. But I was 20 and barely had the money to pay for the trip, let alone miss another day of work and another day of hotel. Plus the Severed Hand opener was perfect for me!
I guess my point is.... sometimes you make shows, sometimes you miss them. The world keeps turning

The other one I regret is Prince’s Piano & a Microphone solo tour. He played Sydney Opera House and the State Theatre in February 2016, just a few weeks before he passed away. I remember being put off by the ticket prices and thinking we’d have another chance to see him.

Near the top of the list is Phish at a small club in Birmingham, AL in 1993. We were going to have a drink beforehand and, once we parked, my buddy who'd driven locked his keys in his car. The printed ticket stubs, unfortunately, were stowed in his glove box. This was before the days of cell phones so there we were standing outside the club, just couldn't go in for the show. To this day we still joke about it.
Another general regret would be obstinately choosing to not like Grateful Dead - live, studio, or otherwise - and therefore not seeing them play during their waning years, which I was around for. Of course, years later they're now firmly etched into my Mount Rushmore of all-time bands and I try to see Bobby, DeadCo, etc as much as possible. Making up for lost time!
