i would imagine marketing firms are probably well aware of the meanings of these things, but they weigh how many people in their client's demographic actually know it. i'm sure there were many fans of the song Born In the USA that had no idea it was critical of the US. many casual fans of big songs like that only know the chorus, and either don't know the rest of the words or they just don't click. when Born first came out, I was one of them.
