I don't get it - why does the NYC option make it sound like I'm buying the book again? What option do I pick if I'm bringing the book?
Same. I did general admission because I thought I'd have to buy the book again. So vague...
It's very vague. Usually book signings u have to buy the book there to get it signed. I took it as the admission was just for the Q&A. I could be wrong....

My take in NYC is if you buy the admission you could bring in the book and you will have a $15 credit for anything else in the store. I would think they are giving this option because there was the pre-sale here and some people got the special edition.  If you got the admission with the book, you're getting admission and the book to be signed at the event.
It's a bit confusing but a nice option for people that already pre-ordered.

And of course I could be wrong

Annnnnd, I was wrong.  I looked under The FAQ at the Strand....

If I already own the featured book and/or have books previously published by the same author/artist, may I bring those to the event to be signed?

No. However, we provide additional copies of the featured book, as well as any previously published books we have in stock, at the event for purchase. Please note that if you do not want to purchase another copy of the featured book, you can always purchase a $15 Strand gift card for admittance to the event.
