No idea... complacency maybe? red tape/chain of command? And just the idea that someone (the president with his thumb up his ass) would have to approve shooting down a commercial airliner is something that would have everyone deferring that decision.
3. Why did the Secret Service allow Bush to complete his elementary school visit, apparently unconcerned about his safety or that of the schoolchildren?
What proof was there that his life (or the kids') was in danger? All of the hijackings/attacks were in the northeast, he was in florida... And unless there is an imminent threat on his life, the decision to stay is squarely the presidents, not the secret service.
