I haven't crowd surfed since 1996. I was watching Rocket From The Crypt at a festival there were loads of people surfing. As I got close to the front the crowd parted like the fricken Red Sea and I hit the deck hard. Before I could get up the crowd surged forward and I got trampled, I was kicked unconscious and cracked a couple of ribs too. It's only because a security guy saw me go down and jumped over the barrier that I'm here now.
One minute I'm being bounced along on the crowd, next minute I'm in the St John's Ambulance room behind the stage. Stage diving went from fun to terrifying in the blink of an eye for me. I don't have a problem with kids doing it at shows now, but I make sure I never drop anyone.

There was crowd surfing in both Manchester UK shows last year
Thankfully they got ejected soon enough.

I go to a lot of club shows with newer bands and kids do crowd surf. I stand in the back with my earplugs in. My days of surfing and being in the pit are over.

Last show I crowd surged at was the second PJ show in seattle in '98. I sort of realized at that point I wasn't a lightweight kid anymore, and was too heavy for crowdsurfing (I was probably 180 pounds at that point).
I agree that for most of us, we go out and watch bands that were big while we were in high school... so the majority of that crowd is no in their 30s/40s. Not really crowdsurfing material.
I'm sure if you went to more recent bands with a younger fanbase you'd probably get some crowdsurfing.
Personally I really hate it now. I'm 6'2 so I'm a little taller than average people height, which means when there are lots of surfers, my head keeps getting sideswiped. It was easier when I was younger and shorter and that didn't happen..
Edit: Last Pearl Jam show I saw crowd surfing at. Calgary 2009. It was a GA festival show, so it wasn't all veteran 10c members up front
Haven't seen it at a PJ show, but I was at a Clutch show a few weeks back (to keep relevant with the age of the band/crowd) and there was crowd surfing. There was a 'real' mosh pit too-the kind where people were jumping up and down and bouncing off each other, there weren't any a-holes with helicopter arms trying to punch everybody.
The joy of life comes from our encounters with new experiences, and hence there is no greater joy than to have an endlessly changing horizon, for each day to have a new and different sun.

Last 2 times I saw Mike Patton perform (FNM in 2010 and Tomahawk in February) he stage dove both shows and crowd surfed a little bit; to get back to the stage.
There was one point at Soundgarden in Toronto a few months ago where throngs of people began to crowd-surf... must've been seven or eight in rapid succession.
