Extraction of wisdom teeth.
Partial bridge story: Mr. Unlost has crowns from breaking his front teeth playing football. One broke, and he was provided with a temporary bridge while the new one was being made. He got up one morning, on his way to present a lecture to a group of firefighters from around the state. Put the bridge down beside the sink. Along came the Late, Great Seamus the Labrador who grabbed the tooth and took off running, chewing it as he went. Mr. Unlost was able to retrieve it, but it was in multiple pieces. He superglued it together and manfully carried on.
Almost as good as when I chipped a front tooth opening a bag of candy and decided to fix it with Superglue as I was on my way to work. Yada, yada, yada, and the tooth fragment was adhered to the Corian countertop in the bathroom, and my upper lip was adhered to my tooth.

4? I dunno, i was offered to get sealants on all my molars when I didn't have any cavities, and my mother said 'they just want more money' so no sealants, and 4 fillings later.... I get sealants when my old ones wear away...I hate getting fillings!
