can someone explain something from the simpsons

sort of a stupid requests, but hey

i was just wondering in the episode where lisa becomes a babysitter and she has taken bart to Dr Nicks hospital she asks Smithers if she can jump ahead of him in the line.

He looks around wierdly and says no. Now im not sure is that meant to be a joke, am i meant to know what he's got or am i jsut raving?
