we had a patient who came in

48 yr old female.
few days ago a patient came in complaining of severe stomache ache.

well, few days prior she had a toothache and decided to not go to the dentist. it gets worse. so then she decided to take tylenol extra strenth. couple of tablets. .. those didnt work. took a couple more. didnt work. repeat.

we figured it out with her. she took about 90 in less than 48 hours. you shouldnt go over 4 grams a day. she took about 22.5 grams in a day. and then again the next day. so she should have been at about 8 grams for two days and she took 45 grams.
so it wasnt a stomach ache. it was her liver that was hurting.
so now her liver is failing. she had no other real diseases. otherwise pretty healthy. she is starting to get fluid belly in her stomach. and starting to show nuerologic damage. she is having a hard time thinking because there is too much nitrogen in her blood stream.
and her kidneys are starting to go next.
its really sad how fast its all happening, and all because of a toothache.

so really people, dont take more than the box says. really really dont.

    sounds like she tried to kill herself, not just the toothache

  • When I worked Retail Pharmacy at Drug Emporium this dude asked me if he had to take a this medicine with food and he's holding the box up

    I said let me see the box it was some kind of cold medicine, I said why did you think you had to take it with food he said because of the shopping cart sticker on the side of the box :p

    The sticker was actually for the alarm to sound at the door if you tried to steal it

    I guess his detergent was Take with Food was well

    Hmmm. what is tylenol? i guess it's name of medication? What is inside, i mean - the medical ingredient?
    Anyway...i know what are U talking about... i have the same problem with my brother -who is so scared of dentist and takes so many ibuprofen )correct?. i try to explain him that overdose has many bad concequences... but his fear is too big

  • I can kind of understand how this happened. She was broke and in pain.

    When I gave birth, I didn't take any medication before during or after. When I had a toothache (that lasted for 2 years until I finally went to the dentist and had a root canal) I would take 4 ibuprophen 3 times a day. I wanted to take more, but I knew it would hurt my liver.

    That lady probably had no idea the harm it would cause. I do understand, though, the pain she was in and how it feels to be broke. :(

    my doctors have always told me you can take up to 6 advil 4 times a day (for when I had menstrual cramps so bad I couldn't function). But I don't think that would be a good idea for extended periods of time.

  • the drug name is acetaminophen.
    but a lot of the pain relievers also have acetaminophen. you might get something that says hydrocodone/apap. the apap is the tylenol. all stuff your pharmacist should tell you about during counseling.

    We've had patients try to suicide by Tylenol od. Really wrecks the liver. But you'd be surprised how most of them bounced back...there is hope for this lady. I think too much Advil harms your stomach, but you need to take a LOT of it. I use Advil Extra Strength Liquigels, but never more than three a day and certainly not two at once...

    After a dentist visit (wisdom tooth extraction that involved cutting the tooth into three), the doc gave me a prescription for ibuprofen 1600mg TID in addition to 15 Tylenol #3. The T3s didn't touch the pain, so everytime I called the dentist he told me to take two more...So, by the time I downed 8 of the damn things, I rapidly experienced every human emotion severall times over and over. Found my way to the hospital and they gave me a giant shot of Gravol in the ass...and then I slept for a couple of days. After all that, the dentist told me to take the T3s AND ibuprofen 1600mg (because my jaw was infected and the swelling didn't go down after four days)...and apparently I made it work (walked) but came home halfway through my shift without telling anyone...my husband found me in a heap behind the couch.

    So yeah -- be CAREFUL with seemingly harmless meds!

    PS, the dentist left town shortly after this incident when he got wind of the fact I was seeking legal counsel and contacting the College of Dentists or whatever (neither of which I did - that was the Tylenol talking!)

  • the lady went into a coma. her kidneys started working again. her liver tests were becoming normal. but her ammonia in her blood was just too high. she passed a few hours later.

    its weird, because it was just a few days ago i was talking to her and now shes gone. sorry. kinda makes me feel weird.

  • Was there ever any discusion why she felt the need to take so many pills? If the pain was that great, Tylenol wasnt really curbing the pain was it? I just dont understand how a reasonable person takes so many pills. Why didnt she throw them up?

    I don't know how many milligrams are in each advil, but I finally had a Gyno tell me very sternly not exceed 2400 mg's a day.

    After all of these years of doing what my physician assistants were telling me to do (which was basically take as many ibuprofen as I wanted), I finally found somebody telling me NO. It can seriously damage your kidneys.

    BTW - The doc who told me to stop taking so much, well his directions actually matched exactly how it reads on a bottle of ibuprofen.

    The maximum dose on the extra strenght is no more than 8 a day and i think they are about 500mg each so yar that's 4 grams. Tylenol is ideal as there is no asprin it, and they are clearly more gentle on our stomachs.

    I'm a critical care paramedic and i've seen more than my share of fatal decisions people make, without understanding the consequences. It's very sad.

    What the hell was she thinking. I'm a fan of reducing the availablity of purchase at any one time (24 in a box, NOT 50 or 100). It's ridiculous. You do not need to have 100 sitting around.

    This is not a completely uncommon occurrance. It happens more often than you might think. Not fatal consequences thankfully, but it certainly can mess you up in a big way.

    I know someone that committed suicide by taking 100 Tylenol and drinking. She knew what she was doing and after years of contemplation I have accepted her decision. She was a relative and I miss her. She was smart, locked herself in a hotel room a few states away and left her notes to everyone.
    She wanted to donate her organs but they couldn't take much, her lenses and long bones I think. Everything else had too much damage.
    Think of her often, and absolutely whenever someone mentions taking a lot of OTC meds I bring up my story of how I never really thought that Tylenol could kill you.

    Suicide by Tylenol is HORRIBLE! It's not immediate. You don't "go to sleep and never wake up". You actually suffer through all your organs failing over the course of a few days. Worse still, if you change your mind and go to the hospital, chances are you're going to die anyway because of the damage that you've caused your liver.
