Guitar Wiring Problems.

ok so ive been working on my strat for a little while and it just not working.
first i do have a decent knowledge of electronics. ive been using the diagrams from the seymour duncan site.
everything looks right, everything checked through when i went through it with a meter.
but still im getting 90% noise and 10% signal.
SO a little background.
i have a jb in the bridge and a single coil in the neck wired into a 5-way switch (neck pickup is wired into the middle slot). from there it goes into one volume and one tone and then to the output jack.
the output jack is grounded to the bridge, everything else is grounded to the pots.
ive seen some other diagrams that have an outside slot of the volume pot go to the ring of the output jack, while others (such as the duncan one) just have it grounded.
any ideas? any of you?
im using 16 AWG hookup wire for everything. and my output jack isnt wired with shielded wire.
im getting some output but the signal is mostly noise. and the signal i have is very quiet.
