What does this mean ?

12 there is an arrow going from 12 out saying full.
15

Should I bend the two strings up a pitch or just one?

Just one, up a full step.

Is it on both strings or just the bottom one?

Yeah like moster78 said, bend only the string that has the arrow next to it a full step. But if the 12 and 15 are on the same beat, you play 12 and 15 (but only bend 12 and full step) at the same time.

I was thinking it was hardly bending the bottom string.

The bottom string shouldn't be bent unless the tabs specifies other than what you posted. It's kind of tricky played more than one string and only bending one.

A semi-circle indicates a hammer on, but from 12/15 to 12/15??? The slashes indicate a slide (in the direction the top is pointing) so perhaps it means slide up (starting at 12/15) then back to 12/15. What is the name of the song?
