Down interpretation anyone?

Down is one of my favorite PJ songs but I still don't get it. I know the line "you can't be neutral on a moving train" is the name of one of Howard Zinn's books, and the song was inspired by him.
But the line "one day the symptoms fade/I think I'll throw these pills away" sounds like it dabbles in Mike McCready's past (with his drug addictions and all.)
That's the only line I understand, does anyone have an interpretation of this song?
