Untitled Bootleg Question

Hello,
I recently purchased a new computer and I'm importing some of my CD's onto its hard drive. I came across a CD wallet in my house that had a Pearl Jam bootleg in it. I got it some time around '97/'98 ..... I only have a photocopy of the back cover of the disc jacket that looks to have an open window on it with the tracks listed to the left of it.
There are 15 tracks on it. The final track is a surprisingly clear version of Yellow Ledbetter that begins with EV saying " we're getting Yellow Ledbetter requests, we could play that" and ends with "see ya tomorrow, good night, thanks."
Does anyone know if my brief description above sounds like any unofficial bootleg that was circulating in the mid-90's? Thank you for your time and efforts.
