This is such an interesting question! I really had to think about it, and these numbers are mostly ballpark cuz don't have exact in front of me, but pretty sure my 5 are:

    1. Pearl Jam: 55+
    2. DMB: 45+
    3. The Frames: 25+
    4. REM: 15+
    5. 3-way TIE: The Clash & Radiohead & Joe Strummer & the Mescaleros: 12

[Edited to add: Just realized I saw Joe Strummer & the Mescaleros 12 times as well, same number as I saw the Clash]

I have no idea how many times I've seen these bands, so in what I imagine to be in order from more shows to less shows:

    Pearl Jam
    Sonic Youth
    Neko Case
Jesse Sykes and the Sweet Hereafter
    The White Stripes

I've also seen Slayer, Springsteen, AC/DC, Neil Young, Faith No More, Jethro Tull, RHCP and Wilco quite a bit.

Band of Horses kick ass and I just was introduced to them (didn't know about them when they opened for PJ and I was sadly in a bar when they played). I can't wait to see them live. Though I did hear Ben play AATWT with PJ in Hartford.

The fact that you've seen Tinariwen and Rachid Taha at all, never mind 3 times each, absolutely blows my mind! Hoping Tinariwen gets to my area sometime on this tour they're doing now...

    Love your list!
