After the chorus do I just play the verse again and then the chorus? Same chords?

The main part that I can't figure out is the "Now the heart is cracked part" - the bridge?

Is it E, F#, A, F#?

I think it may be something more before the E F# part, I think its a C#m chord but not to sure
