Why in the second verse does Eddie sometimes sing Oh dear Jack or I think I have heard John once before as well. Here is an example. http://www.youtube.com/watch?v=AbusoGxK ... AAAAAAABAA Great emotion in the second verse!

Don't know the exact answer, but as Jack is a nickname derivative of John, he maybe still meaning John(ny) Ramone?

I guess only Ed knows the real reason but I would assume that he was paying respects to those who helped him "ride the wave where it takes me....." Jack, who set him up with the demo tape and Johnny who was his dear friend.
It's nice to be nice to the nice.
