Looks like there's no takers, so what are the other three?
In “The Pilot” part 2 when they are showing all the different Seinfeld characters watching the first airing of the “Jerry” pilot, they cut to Newman who is sleeping in a recliner with the Yankee game in the background and the TV announcer says “here comes the 2-1 to Mattingly”.
in “The Engagement” when Jerry and George are on the phone talking about the Yankee game, Jerry says “Mattingly just singled!!” But George has to get off the phone to watch Mad About You with Susan.
In “The Wink” at the beginning of the episode George has an inter office envelope from the Yankees that has a bunch of signatures on it and Kramer asks George “Is that Don Mattingly’s signature?”.

We've done this sort of thing before in this thread. Let's try to name all athlete references. I'll start with five off the top of my head (not counting Mattingly which we've done, and Keith Hernandez/Roger McDowell because that's so obvious) and you guys can add if you want.
-Kramer's altercation with Reggie Miller -Kramer punching Mickey Mantle after giving Joe Peptitone some chin music -"Holyfield. Friend of one of my patients. Has a hell of a body. Ya like him?" -Kramer demonstrates Killer Kowalski's stomach claw move on George in the parking garage -DiMaggio in Dinky Donuts

Wasn't Paul O'neil in an episode with the birthday card and the kid asked him to hit two homeruns?

Yep, the second home run was an inside the park that was changed to a triple and an error. Kramer tried to wrestle the birthday card (which was framed) from a sick kid.
I wish I was a sacrifice, but somehow still lived on.
Jay Buhner, Doug Drabek, and Willie McGee were mentioned when Kramer and Jerry are talking about all the good prospects that George Steinbrenner has traded away.
“How could you give 12 million dollars to Hideki Irabu!!!??!!”
While we're on the subject of Frank's Yankees laments......why did they trade Jay Buhner? He had 30 HRs last year and over 100 RBIs. Has a rocket for an arm. YOU DON'T KNOW WHAT THE HELL YOU'RE DOING!
not necessarily any individual athletes but Comedy Central just showed the episode where George. Elaine, and Kramer went to a Yankees game against the O's and Elaine wouldn't take her O's cap off They did show some players on the tv (couldn't tell who they were though)

“We need more special days, like Joe Pepitone Day or Jon Voight Day.”
Kramer claims Joe Pepitone designed Central Park when steering the Hansom Cab with George’s parents.

Speaking of Buhner, don’t forget Ken Phelps! My baseball people kept saying Ken Phelps, Ken Phelps, Ken Phelps!
Other random mentions that come to mind:
Luis Polonia and Wade Boggs(Cotton is king)
Phil Rizzuto (pothole episode)
Bonds and Griffey (George mentions to Steinbrenner they could trade for both without having to give up much)
John Franco, Gooden, Murray, Bonilla, and Vince Coleman (mentioned on the subway in Jerry’s conversation with the naked guy)
Babe Ruth (was nothing more than a fat old man with little girl legs)
