I just discovered the real answer to the first question never actually aired, but was in the original script. 

and odd, I thought I knew all of the actors that played more than one character on the show; this one stumps me.

For what it's worth, the title of the book is show during the book signing too.

For the second question, I believe he's the cop that turns the siren on for Jerry and George.

I just looked it up. Not sure if it's a script. But there's a picture of the book in the episode. This was a great trivia question. I've probably seen this picture of him holding the book dozens of times and never actually took the time to read it....

I met a SUSIE the other day. Believe it or not she was not a bullemic chain-smoking stenographer from Staten Island.

ALSO....I was up in Long Island for a funeral yesterday. Recently saw the Chinese Woman episode and couldn't get "I'm not taking advice from some girl from Long Island" line out of my head all day. 

AND--my dad successfully utilized the "stop short" reference as he was sitting in the passenger seat when my brother reached over to him when he came to an abrupt stop. Everyone but my dumb brother got the reference. EVEN my mom got it. Very disappointed in him as that's a pretty obvious one.

But...Jackie briefly appears in a 5th episode. What is that appearance?

That’s in season 4. Jackie doesn’t appear until season 7. Here’s a hint, it’s a short scene that the closing credits roll over.

I can't recall the episode, but Kramer is trying to get into his office and Jackie is holding the door shut shouting "no no no no" or something?

It's when Kramer falls asleep on the woman, and then her...friends?....drop Kramer in the river.  She then calls Jackie for legal advice, and he hangs up because he doesn't want nothing to do with it!
