If my days end tomorrow
Will you let me go?
As I fight my hardest to stay here
Would you give me a hand to hold?
No one told me I’d be here this long
Who knew I would be here at all?
But as this all comes to an end
I hope you understand
I never wanted to leave you
You know I hate to see you cry
Now it pains me to see
I’m bringing the tears to your eyes
We had our ups and downs
But as it all comes to an end
Till death do us part
Wasn’t that the plan?

I think this to be brilliant. I get that he is asking her to move on with her life once he's gone even though he doesn't want to leave her. He also needs the favour of having his hand held till death due to the unknown, and he needs her to understand that death comes to us all. He hopes that she'll keep her sanity after he is gone and after the conundrum of death continues to stir in her. Then the second part tells me he leaned over and whispered in her ear to comfort the pain caused in her by his coming death.
Also, I find my interpretation of the poem to be so evident that by writing it here, I am insulting the intelligence of every other person who has read this poem. WOW. That's a first!
