Cycling Survey

My colleagues and I are developing a proposal for a potential client that will include a marketing/PR sponsorship of cycling events. Not sure exactly which ones and where at this time as that will be part of the next planning phase. To get started we are doing an impromtu survey would give us some stats and (hopefully)show support for aligning with the sport.
I was wondering if any of you who cycles at any level would be willing to take the survey and, if possible, forward it to some of your cycling friends. It is brief and non-scientific but we would like a have as many responses as possible.

That's fine...any kind of cycling would apply to this. Road, MTB, cyclocross.
