I am not sure what the "actual" chords are... I found two sources out there:
13inlet's Youtube comments list:
Ultimate-Guitar lists:
I tend to play a mixture, using the Ultimate-Guitar version for the main part of the song, and 13inlet's version of Dm,Dm/F,C,C/B for the chorus...
also, just prior to going into the chorus, I am not sure what chords are repeated, but it does not sound like the entire 4-chord sequence... any pointers?

Ultimate Guitar is accurate. Before the chorus is just a repeat of the 4-chord sequence as you said.0
