I have a tax question - need help!

I received a severance check from my former employer this past July. I was taxed heavily with it. Will I get a separate tax form to file or will it come in the same envelope as my former employer's tax forms for 2009?

    it will be included with the rest of your earnings as part of the standard W2, or at least it should be assuming you were paid via the normal payroll system.

    earnings before seperation + severance = W2 wages from said employer
