Advice please help...

So here's my current situation... and comments will be greatly appreciated...
So over the past month or so I've ran into this girl whom I use to work with years ago a couple of times at mutal friend's houses. I think she is pretty cool and very pretty. I have seen her out at the bars previously and did not approached her thinking that she might not remember who I was and didnt really want to put myself out there at the time. For the most part our conversations have been limited to hey how are doing, that type of scenario. Nothing has really extended into a real conversation until yesterday at her birthday party at one of my friends houses. I've been meaning to play the I think I use to work with you card but waiting for the right time to do it
She arrived with two of her friends, one of which I worked with at the same time and place of this girl. Her friend says "hey I remember you we use to work together" blah blah blah. My response is pretty much oh I don't really remember you but you look familar. So then I play the card I have been waiting to play for a while and turn to her and say " you know a few weeks I realized we use to work together I couldn't remember where I knew you from) I also told her that I had worked at this restaurant for 8 years and there were always a ton of new servers always being hired so I just spaced it.
So it turns out we smoked in my car afterwork one night and I think we might have hung out once or twice but this was in my early 20's I don't really recall much of my college days in detail at this point. My real question for this thread is she completely turned off by me at this point for forgetting everything or do I still have a chance. I want to apologize to her but don't have her number. Is it appropriate for me to ask a friend for it after they ask her if I can have it? or would it be okay to add a friend request on facebook to her and apologize that way, hoping I can score her number at the same time? What do you guys think?

From a friend. They could always talk you up to her and even find out if she might be interested in you. Definitely less creepy that FB :?
