Guitar-related Survey for a School Project

I am working on a final research project for my master's degree. I decided to tie my project to one of my passions...playing the guitar. Part of my research is to run a survey. So, if you aren't to busy and wouldn't mind providing me with your feedback, I would really appreciate input from any of my fellow guitar enthusiasts out there! It should only take 10-15 minutes or so.
